Philippine Passport for New Applicants
Before I enumerate the list of requirements to obtain Philippines passport for new applicants, let me first give you some nice tips to have a smooth and fast process of getting your passport. First, have your documents all consistent and accurate.
Check all important personal details and information and make sure they all tally with your IDs and papers. These details are your complete name, date of your birth, place of birth, your parents’ name, their birthdates, address, and the like. These details must all tally and be consistent because they will be stored in the DFA’s database.
Also please make sure you have all the same signature from your valid IDs to avoid going home. Why you must do this? Simply to avoid embassy problems, government records errors and other related problems.
I also suggest you download the application form and fill it out before you go to your DFA office to complete all requirements and have all data accurate. 3 steps in getting Philippine Passport (New and Renew)
- Scheduling an appointment to your nearest DFA office (please download the DFA application form I linked below to save time in filling it out and falling in line)
- Attending at the exact appointment without being late. (at this time, you must have all the requirements and the fees for passport processing payment)
- Passport releasing

Requirements for Renewal of Passport
Brown Passport or issued prior to 01 May 1995
Please bring the following if they are available.
- Old passport and photocopy of passport pages 1,2,3, 4 (amendment). The pages showing latest Bureau of Immigration departure and arrival stamps.
- Original valid ID.
- Birth Certificate from NSO in Security Paper (SECPA)
- Negative Birth Record from NSO if born 1950 & below and Joint Birth Affidavit from Two Disinterested Persons; and supporting document showing complete Middle Name
- Present passport and photocopy of pages 1,2,3 (amendment) and last page.
- The pages showing latest Bureau of Immigration departure and arrival stamps.
- Photocopy of Birth Certificate for minor applicants.
- For illegitimate minor, personal appearance of mother is required.
- Present passport and photocopy of pages 1,2,3 (amendment) and last page.
- The pages showing latest Bureau of Immigration departure and arrival stamps.
- Photocopy of Birth Certificate for minor applicants.
- Original Birth Certificate from NSO in Security Paper (SECPA) if city/municipality & province on birth place is not specific (Example: NCR instead of San Juan, Metro Manila)
- For illegitimate minor, personal appearance of mother is required.
Please bring the following if they are available.
- Marriage Contract
- PRC / IBP ID
- Land Title
- Driver's License
- Government Service Record
- Digitized SSS-ID
- School Form 137 or Transcript of Records with Dry Seal
- Other documents that show full name and birth details of applicant and/or citizenship
- Voter's Registration Record from COMELEC Intramuros
- Baptismal Certificate w/ Dry Seal
- Seaman's Book
- Income Tax Return (Old)
Requirements for New Applicants
Authenticated NSO birth certificate or Certified True Copy (CTC) of BC issued by the Local Civil Registrar duly authenticated by NSO depending on the NSO signatory; for born 1950 & below, Negative Birth Record from NSO & Joint Birth Affidavit from Two Disinterested Persons if no record from NSO.
Two Original valid IDs (please make sure your signatures are all the same)
Accepted IDs are Senior Citizen's ID, Voter's ID, digitized government-issued IDs like SSS, PRC, BIR, Driver's License, original school ID (for students only) AND original supporting documents indicating full name, date and place of birth and citizenship.
Authenticated marriage certificate (if married or if you want to use your spouse’s surname)
Other supporting documents (for verification)
- Marriage Contract
- Land Title
- Driver's License
- Government Service Record
- School Form 137 or Transcript of Records with dry seal

UPDATE Jan. 25, 2012:
Good news to everyone! The Department of Foreign Affairs announced
recently that we can process our passport renewal in some malls
nationwide soon. I’ve seen a big billboard from a huge mall (Robinsons
Starmills) in my province already and this is very good news because we
don’t need to go to any DFA office that is far from our home town. At
least it’s gonna be more convenient and accessible. Plus it will lessen
headaches coz the malls are cololer than some DFA office service areas.
While only renewing of passports are gonna be entertained, apparently
soon new passport application service will follow.
